When an Eye Examination Reveals More Than Expected

Diane, a 17-year-old high school student, attended our practice because she was experiencing difficulty concentrating while reading, significant light sensitivity and problems maintaining focus during her schoolwork. Despite previous eye examinations confirming that she did not require glasses, her symptoms persisted.
During her comprehensive assessment, her eyesight was found to be essentially normal, and her symptoms were consistent with Visual Stress Syndrome, a condition that affects the way the brain processes visual information rather than the sharpness of vision itself.
As part of her examination, retinal imaging was performed. While assessing the back of the eye, an unexpected pale lesion was discovered in the retina of her right eye. Although the lesion appeared flat and had reassuring features, it was unusual enough to warrant referral to a retinal specialist for a second opinion.
Further assessment by ophthalmologist Dr James Chau-Vo confirmed the diagnosis of a choroidal osteoma—a rare, benign (non-cancerous) tumour in which mature bone develops within the choroid, the vascular layer beneath the retina. Choroidal osteomas most commonly occur in healthy young females and are usually found in one eye. While they are not cancerous, they require ongoing monitoring because, in some cases, they can slowly affect vision over time or lead to secondary retinal complications.
Fortunately, Diane’s vision remains good, there is no evidence of significant visual field loss, and the lesion will simply be monitored with regular retinal imaging and review. Home Amsler grid monitoring has also been recommended.
Why This Case Matters
This case highlights why a comprehensive eye examination is about far more than determining whether someone needs glasses. Many serious or uncommon eye conditions produce no symptoms at all and are discovered only through careful examination of the retina.
Early detection allows appropriate referral, confirmation of the diagnosis and ongoing monitoring, providing reassurance for patients while helping to preserve vision if changes occur in the future.
